Clinic Connectome
Home-visit medicine asks a small number of clinicians to travel across a wide area and care continuously for a large number of patients. Documentation, reimbursement coding, and coordination with nursing, care management, and pharmacy consume time that should go to patients.
Clinic Connectome is the platform we build in-house to reduce that load. It does not replace the electronic health record; it sits alongside it and supplies what home-visit practice needs and the EHR does not.
- Reimbursement compliance Checks clinical documentation against Japan's home-visit reimbursement rules. Every determination maps to an explicit written rule, so a human can trace why a conclusion was reached.
- Telehealth and clinical dictation Self-hosted video consultation, with speech recognition that turns the consultation into a draft record.
- Care coordination Brings each patient's history, messages, and input from other professions into one place, so care survives a change of staff.
Technical approach
Patient data does not leave our network. All processing, including language model inference, runs on GPU servers installed in our own facilities rather than on external cloud services. This is the reason the platform is built the way it is.
For reimbursement decisions, we do not treat a language model's output as the answer. Model-based extraction is paired with a layer in which the rules are written out explicitly, because any judgment that affects a medical claim has to be defensible.
The platform is in daily use across Shinshinkai and the group clinics. It is not offered to outside organizations.
